Will I automatically receive Form 1095? 
The City of Lobelville will provide Form 1095 upon request. Employees may request a copy at any time. 
Do I need Form 1095 to file my federal income tax return? 
According to the IRS, you generally do not need Form 1095 to file your tax return. You should keep it with your tax records. 
How do I request a copy of my Form 1095?
